The Journey Home is accepting applications for Full-Time Case Managers for the first and second shifts who are well-organized, can manage multiple priorities effectively, and communicate clearly. As members of the client services team, Case Managers work collaboratively with other staff members and community partners to deliver resources that provide a holistic approach to client care and maximize the community-driven problem-solving present in Rutherford County. Case Managers will build professional relationships, administer assessments, collaborate on plan development, housing search and application process, placement, monitoring, and service coordination.
